Externship Requirements
Prior to starting on-site externships, you’ll need to complete requirements for health and safety required by your employer.

Proof of COVID Shot
Deadline: Wednesday, July 12th
You must provide proof of being fully vaccinated:
Option 1: 2 doses of Moderna/Pfizer or 1 dose of Johnson & Johnson and 1 Booster Shot
Option 2: 1 dose of Bivalent Shot
You can use this website to locate vaccination options in your area.

Immunization Records
Deadline: Wednesday, July 12th
In order to remain in the program you will need to submit proof of each required immunization. Contact your Primary Care Physician to ask them to send you your immunization records OR request your immunization records from your state health department.

Background Check
Deadline: Wednesday, July 12th
You should have received an email communication (to the email from your Propel application) with instructions for completing your background check.

Fingerprinting Appointment
Deadline: Wednesday, July 12th
You should have received an email communication (to the email from your Propel application) with instructions to schedule fingerprinting. Sign up for your fingerprinting on IdentoGo.

Child Abuse Clearance
Deadline: Wednesday, July 12th

Drug Test
Deadline: Wednesday, July 12th
You should have received an email communication (to the email from your Propel application) with instructions to schedule your drug test at QuestDiagnostics.

Proof of Flu Shot
Deadline: Wednesday, September 20th
For externships, all fellows must receive a flu shot. The 2023 flu shot will be available starting in September.

Fingerprinting Results
Deadline: Wednesday, September 20th
After you complete fingerprinting, it will take up to 6 weeks to receive the results in the mail. Once received, take a picture of the results and upload it below.

TB Test
Deadline: Wednesday, September 20th
For externships, all fellows must complete a 2-Step PPD or Quantiferon Gold blood test.
