Externship Requirements

Prior to starting on-site externships, you’ll need to complete requirements for health and safety required by your employer.

Due Date: Sunday October 16th

Please Submit now if you have not already!

Please reach out to your coach if you have something preventing you from moving forward with these requirements.

  • This visit needs to take place before November 13th. The doctor must fill out the form here.

    If you don’t already have a primary care provider, you can find one via zocdoc.com or freeclinics.com.

  • If this if your first shot, you will need to receive and upload the first shot by Oct 16th.

    You can submit your COVID card or upload this as a part of your immunization records. If you are in Camden, you will need to also get the COVID booster as required by Cooper University Hospital.

  • Immunization records must show proof of the following vaccines:

    Varicella/chicken pox (2 doses)

    MMR (2 doses)

    Hep B (3 doses)

    TDap (1 dose)

Proof of Health Insurance

Many of our externship sites require that anyone serving in an externship at their site has insurance to protect themselves.

  • Start to talk with your family and do research on your options to secure coverage. We understand that finding and paying for health insurance can be a challenging process. Please consider the following resources:

    • Healthcare.gov: This is the open marketplace available through the Affordable Care Act. Open enrollment runs from Nov 1 - Jan 15th. 

    • State-based exchanges: This website lists the states that have their own insurance exchanges as well. Scroll down on the page to identify if your state has a program.

    2. TB test

    3. Remaining vaccinations (if needed)
